How can we help...
Our therapy professionals can help with a variety of needs aimed at improving your independence, strength, safety, and quality of life within the context that is most meaningful to you. Your physician may make a referral to outpatient physical or/and occupational therapy services as part of a comprehensive care plan if you:
· Have had a recent fall and would benefit from a home safety assessment and fall prevention interventions to reduce risk of future falls.
·. Are living alone and are having difficulty getting around and a fear of falling or history of falls.
· Are recovering from an injury, surgery, or illness and need help returning to your prior level of functioning and are not home-bound.
· Would benefit from gaining knowledge about adaptive equipment, strategies, or tools to facilitate improved functioning with everyday tasks.
· Could use help with the management of symptoms resulting from chronic or neurological disease such as stroke, COPD, Multiple Sclerosis or Parkinson’s.
· Are you a caregiver who would like to learn strategies for increasing independence and providing a more safe environment for your loved one.

We are the innovative leader in providing outpatient therapy services including physical therapy and occupational therapy. Therapy is provided where it matters most, at home. Nearly 90% of people want to stay in their homes as they age or recover from illness or injury, but if they don’t qualify for home health services, they feel their options are limited in how to achieve that goal.
